Driver's Accident Report

The Driver's Accident Report is designed to assist your drivers in collecting appropriate information in the event of a vehicle accident. This information can be used to support your company and Westfield in better managing and evaluating claims to control cost and limit future occurrences. For best results, double side print so that both pages are on one sheet of paper. Fold the sheet in half lengthwise to create a report form that can be easily stored in the glove box or other area of the vehicle.
